If you require any vaccinations relating to foreign travel you need to complete the Holiday Health Planner application form below in order to make an appointment with the practice nurse to discuss your travel arrangements. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.

It is important to make this initial appointment as early as possible - at least 6 weeks before you travel - as a second appointment will be required with the practice nurse to actually receive the vaccinations. These vaccines have to be ordered as they are not a stock vaccine. Your second appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.

To help us offer the appropriate advice, please fill out our travel advise form and return it to the surgery before booking an appointment with our Practice Nurse Team.
Please remember we need a MINIMUM of 6 week's notice before your departure date to assess your travel form, advise you of your vaccination needs and arrange appointments if appropriate. Failure to give appropriate notice will mean we are unable to offer you this service and you will need to make alternative arrangements with another Travel Clinic.
Providing travel advice and treatments are not covered under our NHS contract. Fees will be applicable for any treatment given.

You can then either: return the form to the reception or e-mail it to email@example.com This will give the nurse the information required to check the latest requirements. The nurse will then contact you to make an appropriate appointment.
While a letter may be required for travel, this is not an NHS service. Primary care can charge patients for the time taken to write the letter, or can decline to provide a letter and direct patients to private travel clinics for this service. At the present time Freshwell Health Centre are not providing this service to allow our clinicians to concentrate on NHS patient care and managing the Coronovirus pandemic.
