BANK HOLIDAY CLOSURE

The Surgery and Dispensary will be closed on Monday 2nd May for the May Day Bank Holiday.  We will re-open as normal on Tuesday 3rd May.  If you require medical assistance whilst we are closed, please call NHS 111.  If you have a life-threatening emergency, please dial 999. 

CLOSED FOR STAFF TRAINING

The Surgery and Dispensary will be closed on Thursday 19th May 2022 from 12.30pm for staff training due to an MECCG Shutdown. Please call 111 if you require medical assistance on that afternoon.

Please collect any medication in the morning, Thank you.

The surgery will resume normal service at 8.30am on Friday 20th May 2022

Please note that this is for medical data which may be shared with third parties. Your Summary Care Record will not be affected by this and will still be shared with hospitals and other NHS care providers only, unless you want to opt out from that too. Collection of GP Data NHS Digital released a statement last month regarding the launch of the new system they will be using to collect GP data, called the General Practice Data for Planning and Research (GPDPR) service. The statement highlights that NHS Digital will only share data with organisations who have a legal basis and meet strict criteria to use it for local, regional and national planning, policy development, commissioning, public health and research purposes. They have also developed a public advice webpage to explain how and why the data is being used.

Travel Vaccinationsglobal air travel

If you require any vaccinations relating to foreign travel you need to complete the Holiday Health Planner application form below in order to make an appointment with the practice nurse to discuss your travel arrangements. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.

It is important to make this initial appointment as early as possible - at least 6 weeks before you travel - as a second appointment will be required with the practice nurse to actually receive the vaccinations. These vaccines have to be ordered as they are not a stock vaccine. Your second appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.


Travel Health Questionnaire

To help us offer the appropriate advice, please fill out our travel advise form and return it to the surgery before booking an appointment with our Practice Nurse Team. 

Please remember we need a MINIMUM of 6 week's notice before your departure date to assess your travel form, advise you of your vaccination needs and arrange appointments if appropriate.  Failure to give appropriate notice will mean we are unable to offer you this service and you will need to make alternative arrangements with another Travel Clinic.

Providing travel advice and treatments are not covered under our NHS contract.  Fees will be applicable for any treatment given. 

Holiday Health Planner

You can then either: return the form to the reception or e-mail it to freshwell.administration@nhs.net  This will give the nurse the information required to check the latest requirements. The nurse will then contact you to make an appropriate appointment. 


Travel Letters
While a letter may be required for travel, this is not an NHS service. Primary care can charge patients for the time taken to write the letter, or can decline to provide a letter and direct patients to private travel clinics for this service. At the present time Freshwell Health Centre are not providing this service to allow our clinicians to concentrate on NHS patient care and managing the Coronovirus pandemic.
