Appointment Pressures

We are experiencing exceptionally high demand for our GP appointments and we know this has a knock on effect for you our patients.

To help us help you we would encourage all patients to utilise our eConsult service available via this website, to confidentially contact the practice with routine and urgent but not medical emergency queries.

This service has been designed to help your GPs manage and assess your healthcare needs efficiently.  If you use this service before 2pm we aim to contact you back the same day.

Thank you for your ongoing support and understanding.

EMAIL SCAM WARNING

We have been alerted by patients that there is an email in circulation claiming to be from the NHS asking you to book a COVID Vaccination.  They seem genuine in appearance but are asking for your bank details and a payment.

COVID Vaccinations are FREE, the NHS will never ask you for a payment for this.

Please stay safe online, if you are suspicious of any email please report it to your service provider or the police and block further emails.

Please speak with your family and friends if you think they may be at risk from this or other types of email scams, encourage them to speak out before signing up.

Communications and Consent Form

If you would like online access use this form: Online Access Request Form

Please also read this information on Proof of Address and the ID we require:  New Patient ID Check

We would prefer you not to come in for the time being to register as it will expose you and us to the risk of contracting coronavirus.

If you live in the practice area and you would like to become a patient of the practice, you will need to visit the reception in person to collect a form called a GMS1 which you should complete and return to the reception, along with photographic ID for EACH person registering and one proof of address per person aged 18 years and over.  When registering a child you will need to provide proof of name such as the child's Red Book, Medical Card or new baby NHS number & discharge details.

Once you have returned the form it will be given to one of the doctors who will sign it agreeing to accept you as a patient. If the doctor refuses to sign the form (The most likely reason for this would be that you do not live in the practice area.) then we will not be able to accept you as a patient of the practice but we will help you with any urgent medical attention you require.

Once you have registered with the practice Primary Care Support England (PCSE) will be informed and your medical records will be requested from your previous GP. Please note that it may take several weeks for us to receive your old medical records.

Please note that you may see any doctor within the practice not just the doctor you are officially registered with.
